WAIVER

I acknowledge that Local Dance Co Balmain will not be held responsible for any illness, personal injury or adverse change in medical condition or state of health arising directly or indirectly from any participation in training, lessons, course or program that me or my child undertakes whilst enrolled or on premises at Local Dance Co Balmain.

These activities entail known and unanticipated risks that could result in physical or emotional  injury or damage to myself, to property, or to third parties. I understand that  such risks simply cannot be eliminated without jeopardizing the essential qualities of the activity. 

I have explained to my child/children the behaviour that is expected of them to ensure their safety at Local Dance Co Balmain. I take full responsibility of my child’s conduct and actions whilst at Local Dance Co Balmain. I understand that I will be financially responsible for any damages of Local Dance Co Balmain property and/or equipment incurred by myself or my child whilst on premises at Local Dance Co Balmain.

I acknowledge that refunds are not available or applicable in regards to me and/or my child’s tuition if classes are missed and/or participations stops.

I acknowledge that Local Dance Co Balmain has disclosed to me that I and my child/children may be photographed, filmed or otherwise recorded whilst participating in the Local Dance Co Balmain program, concerts and related activities and consent to this.

Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behaviour in addition to any damage I/we may cause to the facilities utilised by Local Dance Co Balmain.
